First go to Manage Assessment Templates in the assessment menu

Then Click the Import assessment button from CSV

Select the file you want

After it is loaded, select Load and Map Columns

Make sure to fill out the Lifecycle Insights Field and try to match the Column Header from file.

At the bottom of the page click Import Data, and then you may edit these as if you had just created an assessment.
